The developing vasculature of the skin is comprised of microvessels that invade the dermis during puberty and are pruned out by the immune system during the first 6 years of life. In the adult, the microvasculature of the skin is largely provided by the vascular plexus in the connective tissue septae. This plexus is highly sensitive to a variety of stimuli and is the major route of blood flow to the dermis. The microvascular plexus also supplies oxygen to the basal epidermal layers and is thus of fundamental importance in the functional architecture of the skin. Various conditions that compromise the integrity of this microvascular network, for example, edema and trauma, lead to subepidermal blister formation. In contrast, skin diseases that are characterized by the deficiency of septal blood flow, for example, the acute venous insufficiency syndromes, are associated with subepidermal blisters.
